For your convenience, you may print and complete the Medical History Form to bring to your appointment. If you have appointments scheduled for multiple patients, one form per patients is required.

Please note: Credit/Debit card payments processed through the terminal in office will have a 4% transaction fee added to the amount charged. Payment methods exempt from the transaction fee include: Cash, Check, HSA & FSA cards.
